Video Summary
The webinar examined regulatory and reimbursement challenges facing inpatient rehabilitation facilities (IRFs), with a focus on Medicare audits, coverage rules, and Medicare Advantage (MA).

Presenters reviewed a recent nationwide Office of Inspector General (OIG) audit. Although the OIG found substantial documentation-related error rates, it found no fraud. After clinicians and rehabilitation organizations reviewed selected cases with the OIG and CMS, CMS agreed with stakeholders on most disputed examples and said the OIG’s findings and perceived risks may have been overstated. Speakers urged clearer review guidance, physician involvement in medical-necessity denials, and greater attention to the full clinical record.

The panel discussed new rules requiring all ordered therapies to begin within 36 hours and the initial interdisciplinary team conference to occur by the fourth day of admission. Clinicians raised concerns about administrative burden, disrupted care, and reduced availability for other patient services. CMS temporarily exercised enforcement discretion for the conference deadline, while stakeholders continue to seek changes.

The IRF Review Choice Demonstration is expanding to additional states and requires review of every claim. Speakers described its operational burden and questioned its value given high affirmation rates. They also reported strong opposition to proposed IRF payment reforms that could reduce complex patients to a single diagnosis or inadequately account for comorbidities.

Finally, OIG reports found wide variation in MA prior-authorization decisions and frequent reversals of denials on appeal. The panel emphasized that MA plans must follow Traditional Medicare coverage criteria, encouraged providers to appeal inappropriate denials, and recommended clearly documenting the need for intensive interdisciplinary rehabilitation and rehabilitation-physician care.
