Pdf Summary
The study investigates the impact of preoperative muscle strength on persistent pain following hip arthroscopy for femoroacetabular impingement syndrome (FAIS). FAIS is a common condition causing hip and groin pain, with hip arthroscopy serving as a surgical treatment option when other treatments are ineffective. This research is a retrospective cohort study conducted at Kitasato University Hospital, Japan, involving 78 patients who underwent hip arthroscopy for FAIS between December 2015 and September 2021.

The main objective was to determine if preoperative muscle weakness in the hip abductors and quadriceps is associated with persistent pain and poor functional outcomes one year after surgery. Researchers found that 20.5% of patients experienced persistent pain post-surgery. Those with low preoperative strength in hip abductor and quadriceps muscles were more likely to have ongoing pain and lower scores in the Japanese Orthopaedic Association Hip Disease Evaluation Questionnaire (JHEQ) used to assess patient-reported outcomes (PRO).

Preoperative muscle weakness emerged as a significant risk factor, suggesting that enhancing muscle strength before surgery could potentially improve postoperative outcomes. The results indicate a need for pre-surgical rehabilitation to address these muscle weaknesses, possibly reducing postoperative pain and improving the quality of life and satisfaction among patients.

Despite its findings, the study has limitations, including the potential for selection bias due to its retrospective design, a relatively small sample size, and the fact that it was conducted at a single center with procedures performed by one surgeon. It also didn't measure contralateral muscle strength or control for the intensity of home-based postoperative exercises. The study underscores the importance of assessing preoperative muscle strength and integrating targeted prehabilitation to improve surgical outcomes for FAIS.
