Description

Age-friendly care initiatives are vital strategies to improve healthcare services and patient outcomes. Rehabilitation principles align with age-friendly care, focusing on what matters to them, multiprong approaches, consideration of balancing measures, and functional independence. Physiatrists are well equipped with knowledge and skills to implement age-friendly care across all spectrums of healthcare delivery systematically. This session will discuss the implementation process in various care settings (e.g., consultation services in acute care hospitals, inpatient rehabilitation, and outpatient clinics) amongst a panel of community members who successfully implemented age-friendly care.
