The implantable Brain-Computer Interface (BCI) sector is rapidly transitioning from the laboratory to the clinic. As companies enter pivotal trials for devices capable of restoring communication and high-resolution motor control to patients with severe neurologic impairments (e.g., C-spine SCI, ALS, Brainstem stroke), our specialty faces a critical clinical infrastructure gap. This session serves as a national clinical briefing on the current hardware landscape (differentiating penetrating, surface, and endovascular arrays) and outlines the "Implantation-to-Independence" clinical pathway. Attendees will learn how to phenotype patients for BCI candidacy, manage acute post-operative neuro-prosthetic integration, and operationalize the lifelong management of the human-machine interface.

Learning Objectives

• After completing this live activity/enduring material, the participant should be able to: Differentiate the current clinical landscape of implantable Brain-Computer Interface (BCI) modalities, including penetrating, surface, and endovascular delivery systems.
• Define the physiatrist's specific clinical role in the four-phase "Implantation-to-Independence" BCI pathway, from candidate phenotyping to chronic interface management.
• Describe the institutional "site-readiness" requirements necessary to establish PM&R-directed BCI clinics and host multicenter corporate trials.
